Do you see one of our network civilian providers? If so, you may be one of many active duty members affected by the recent issue of improper reporting procedures of medical documentation. 

Your job, as an active duty member, is to be aware of the proper processes and procedures in place to ensure your medical records are up to date. 

When an active duty member is seen at a military facility, medical documentation for that visit is uploaded electronically. However, when a member goes to see one of our civilian providers, either by referral or on their own, there is a separate process followed to make sure the documentation is reflected in their military record. 

If any diagnosis changes or medication is prescribed by a civilian provider, the active duty member must contact their Primary Care Manager as soon as possible to ensure the changes are documented properly within their medical record. If disregarded, important medical information kept from your PCM may affect future care. 

Keep your PCM aware of any and all changes made by a civilian provider to ensure you receive the highest quality care.
